To register with Corpus Christi Parish, please fill out the following forms. Forms are also available in the parish office and in the back of the church.
Please contact the church office (719-633-1457 ) concerning any changes in your life. This includes changes in address, phone, marital status, births, deaths, if you have changed parishes, etc. This will enable us to keep our records up-to-date. Some of our documents and forms are stored in Adobe PDF format. If your browser cannot display them, this link will describe how to get the required plug-in for your browser. Since the download for Adobe is very large, we will also provide alternative viewing formats where possible.
